افزایش تداخل شناختی در تکلیف شبه استروپ بزرگ-کوچک در میان کودکان مبتلا به اختلال هماهنگی رشدی: پیامدی برای پیش‌بینی و شناسایی زودهنگام

چکیده
هدف: اختلال هماهنگی تحولی حدود ۵ تا ۶ درصد از کودکان را تحت تأثیر قرار می‌دهد و پیامدهایی فراتر از نقص درمهارت‌های حرکتی، از جمله نقص در کنترل مهاری، به همراه دارد. یافته های متناقضی در مطالعات مربوط به مبادله سرعتدقت، در این کودکان گزارش شده است.
روش: در این مطالعه از نمره کارایی معکوس که زمان واکنش و تعداد خطاها را با هم ترکیب می‌کند و نیز تحلیل منحنی ویژگی عملیاتی گیرنده که دقت تشخیصی را به صورت کمی اندازه‌گیری می‌نماید برای ۲۲ کودک مبتلا به اختلال هماهنگی تحولی و ۲۶ کودک دارای رشد بهنجار در رده سنی ۷ تا ۱۰ سال، طی یک تکلیف شبه‌استروپ بزرگکوچک استفاده شد و توانایی تمایز این شاخص‌ها مورد ارزیابی قرارگرفت.
یافته­‌ها: کودکان مبتلا به اختلال در هر دو شرایط همخوان و ناهمخوان، زمان پاسخ طولانی‌تر و خطاهای بیشتری داشتند و همچنین هزینه تداخل و شاخص امتیاز کارایی معکوس در هر دو شرایط در گروه دارای اختلال بالاتر بود. تحلیل منحنی ویژگی عملیاتی گیرنده نشان داد که شاخص امتیاز کارایی معکوس ناهمخوان بالاترین دقت تشخیصی را دارد. همچنین زمان پاسخ در شرایط ناهمخوان، تعداد خطاهای ناهمخوان و شاخص امتیاز کارایی معکوس همخوان نیز مقادیر سطح زیر منحنی بالای 80/0 را نشان دادند.
نتیجه­‌گیری: نتایج نشان دادند که کودکان مبتلا به اختلال هماهنگی تحولی در مقایسه با کودکان دارای رشد بهنجار، عملکرد ضعیف‌تری در تکلیف مرتبط با کنترل بازداری نشان دادند. شاخص نمره کارایی معکوس در شرایط ناهمخوان، بالاترین توان تمایز را در میان شاخص‌های بررسی‌شده نشان داد و می‌تواند به عنوان یک شاخص امیدوارکننده برای بررسی‌های بیشتر و مطالعات اعتبارسنجی آینده مطرح شود. این یافته‌ها بر اهمیت ارزیابی کارکردهای اجرایی در کودکان دارای اختلال هماهنگی تحولی تأکید می‌کنند، هرچند تأیید کاربرد بالینی این شاخص مستلزم مطالعات با حجم نمونه بزرگ‌تر و نمونه‌های مستقل است.
